All young adults are invited to join us for our co-ed Turkey Bowl Flag Football game. Whether you’re home from college, new to the area, or have never attended a Young Adult event before, come enjoy a fun game of flag football, fellowship, and community as we give thanks to God for the blessings of faith and friendship. No experience is necessary, but all participants must complete the registration form prior to the event. All are welcome!

Assumption of Risk and Release of Liability(Required)
I understand that participation in the St. James Young Adult Ministry Turkey Bowl Flag Football event involves physical activity and carries inherent risks, including but not limited to falls, collisions, strains, sprains, and other injuries. By signing below, I voluntarily choose to participate in this event and assume all risks associated with my participation. I release and hold harmless St. James Parish, the Archdiocese of Chicago, event organizers, volunteers, and affiliated individuals from any liability, claims, demands, or causes of action arising from injury, illness, loss, or damage that may occur as a result of my participation. I certify that I am physically able to participate in recreational flag football activities and understand that I am responsible for my own health and well-being during the event.

Medical Treatment Authorization(Required)
In the event of an emergency, I authorize event staff and emergency medical personnel to obtain medical treatment on my behalf if I am unable to provide consent. I understand that I am responsible for any costs associated with such treatment.

Photo Release(Required)
I grant permission for St. James Parish and the Young Adult Ministry to use photographs and video recordings taken during the event for promotional, marketing, social media, and parish communication purposes without compensation.

Code of Conduct(Required)
I agree to conduct myself in a respectful, sportsmanlike, and Christian manner throughout the event and to follow all instructions provided by event organizers.
